Health Insurance:

Major Medical & Dental Eligibility:

Coverage is available for the individual, spouse and child(ren). After the 1st of the month following 30 days of employment, Dan contributes on the following schedule:

After the 1st month Dan's will pay 25% of your premium.

After 2nd month: 25% of premium

After 3rd month: 50% of premium

Vision Plan:

Dan will contribute annually to the cost of an eye exam and new lenses for all lab staff and bi-annually for all other staff members.

Sick Days:

After 6 months of employment three sick days are earned.

Short Term Disability Insurance and Life Insurance:

Employees are eligible on the first of the month following one month's employment.

Short Term Disability Insurance is available for $7.00 per month. Benefits will be paid after the first day of an accident or after the eighth day of an illness. 66.67% of your wage/salary will be paid up to 26 weeks.

$15,000 life insurance policy will be provided by Dan's at no cost to the employee.

Bereavement Policy:

3 days with pay for spouse, child, parent or sibling.

Vacation Days:

After six months of employment: five days per year

After two years: eight days per year

After four years: ten days per year

After eight years: fifteen days per year

After fifteen years: twenty days per year.

Extra vacation days can be earned by taking vacations in off-peak times.

Holidays:

8 hours of pay for these six holidays, even if the holiday falls on a Sunday:

New Year's Day, Memorial Day, 4th of July, Labor Day, Thanksgiving, Christmas.

Monthly bonus based on profitability and productivity.

Because staying current with technology is critical to our success, Dan's will assist employees in purchasing a personal computer.
